
There is a phrase Dr. Agullo repeats to pretty much every Brazilian butt lift patient at Southwest Plastic Surgery, mostly while they are still in the consulting chair, and basically it is: “This fat is alive, and the first two weeks are where it all counts.”
It is all the surgeon, right? But surgery is just one side of the coin. The other half happens in our treatment room, on massage tables, under compression, on our booty pillow. This is that other half.
The Two-Week Rules
Fat transfers rely on their ability to connect to an existing bloodstream during the first few weeks, with the main risk to cell survival coming from pressure; thus, all of the rules of post-operative care revolve around not applying too much pressure to the fat transferred.
He is strict on this one. No sitting on hard surfaces for two weeks. Instead, all of his patients sit on what he calls a BBL pillow or booty pillow, placing weight solely on the thighs, allowing the buttocks to remain floating free.
Honestly? Sleeping on your tummy is healthiest for you; if an operation prevents you from doing that, then they give you these little wedges that keep your upper half tilted back in a beach chair supported on your back, so your weight does not push down on the butt. Try that at least two weeks straight before retiring those restrictions.
What the Garment Is Actually Doing
The patient wakes up after surgery, the areas liposucked wrapped in an article of clothing called a compression garment; they will wear it for the first weeks following surgery. The article of clothing is not just decorative; it limits swelling in the fat donor areas (often flanks, abdomen, hips), supporting skin in its effort to contract back over the reduced volumes while retaining curves to prevent it appearing as merely a bump on a flat body.
Quality matters and Dr. Agullo does not take that gamble on behalf of his patients. He has studied the garments himself and makes certain that patients are outfitted with high-quality pressure-set compression garments provided by the office, not links to a marketplace.
The Massage Schedule That Separates Smooth From Lumpy
If you asked Dr. Agullo to summarize the absolute single best piece of BBL recovery advice and the answer was not the special BBL pillow, it was the massage table.
In the first month of treatment, Manual Lymph Drainage (MLD) was two to three times a week, encouraging patients to increase the number of massages if they feel comfortable doing so in this first four-week period. Massage is the breakdown of scar tissue (fibrosis) in the areas of liposuction and moves retained liquid out of the tissues and back into the bloodstream. MLD is the difference between smooth curves and irregularities which nobody wants.
Our postoperative program focuses on lymphatic drainage after liposuction or fat grafting, a service we provide routinely and which is coordinated directly with the surgeon for each individual case to stay up to date on the patient’s recovery.
The Timeline Patients Actually Live
Patients describe that first-session-back-at-the-gym feeling: sore with every movement, but nothing they cannot push through. That soreness is pretty much gone after five to ten days. The vast majority are back on a light duty job five to seven days post-op, right around when the drain comes out.
Traveling patients fly home on day 5. They may sit normally from the 2-week mark onward and have full clearance, no restrictions, from 4 weeks out. The volume you see at that 4-week mark is basically the volume you keep. By 3 to 6 months you know the final result.

After the First Month: Keep the Scale Steady
Once the fat has endured its initial 30 days, it works like fat anywhere else. Losing weight means shrinking fat; gaining means it swells and tugs on skin. But it is the up and down cycling (yo-yo weight cycling) which causes the biggest problem: it will stretch the skin out over time, invite sagging, and lead to contour problems that are difficult to improve.
Warning signs are few but vital: an infection showing as increasing redness, cloudy or foul discharge; a focal point of extreme, escalating pain; or a fluid collection (a seroma) in one of your liposuction sites. If any of those occur, get on the phone and ring our office that same day, no need to wait or monitor.
