What the National Plastic Surgery Numbers Missed About Our Region

The American Society of Plastic Surgeons released its 2025 statistics, and the coverage has been uniform. Liposuction is still number one. Eyelid surgery is surging.

All true. All national. Dr. Frank Agullo read to the regional tables, where five procedures moved against the national number in the eight-state region that includes Texas.

The Brazilian butt lift, down nationally and up here

“There is a tendency toward the more natural look, so there has been a decrease in the number of BBLs nationally,” Dr. Agullo says. “But at the same time, a lot of patients are turning to fat transfer or donor grafts instead. In the Southwest, there’s still a demand for the bigger hips and greater projection.”

He does not think the regional number is only local taste.

“Although there may be a decrease nationwide, it may be that patients are seeking out experts in the field and coming to the South and Southwest for these procedures. They’ve done their research. They’re also seeking physicians who use ultrasound for the fat transfer to avoid the gluteal complications.”

Why liposuction stays at number one

Liposuction has led the country for five straight years, and actually dipped 1 percent this year.

“Liposuction is certainly the most common procedure in my practice, and the main reason is that it’s part of any body contouring procedure. When I perform a tummy tuck, liposuction is included in the abdomen and the lower back. When I perform an arm lift, I do liposuction first. When I do a thigh lift, I do liposuction first. When we perform many breast procedures, we’re often doing liposuction of the lateral chest, the axilla, or the upper back.”

“Therefore, liposuction really is a very common procedure in the practice, well over a thousand cases per year.”

The GLP-1 finding that is not in the report

The lower body lift, the major operation after significant weight loss, grew 4 percent nationally and fell 14 percent in our region. Dr. Agullo was surprised.

“I’m also surprised at this number, since we perform it quite often. The lower body lift is the key surgery after massive weight loss, and I’ve been seeing a high volume of this procedure in our practice.”

“What I’ve seen in my practice is a lot more patients coming in for facial surgeries after massive weight loss with GLP-1 medications like semaglutide and tirzepatide. The lower body lift numbers may have decreased because patients are more concerned with the facial aspect first and may pursue the body contouring later.”

Face first, body later. If that is right, those procedures are not gone. They are waiting behind a wave of facial surgery.

Dr. Agullo has published the full breakdown, including rhinoplasty, gynecomastia, and what these numbers do not count.
